Norfolk Medical Group and Heartland Pediatrics are offering Sports Physicals.
These will be offered for students going into the 8th grade through Senior High.
Date: Friday, May 31st and Friday, August 2nd
Time: 8:00 a.m. to 3:30 p.m.
WHERE: Norfolk Medical Group Pediatrics Department located at Fountain Point Medical Community, 3901 West Norfolk Avenue.
You must bring with you:
****Sports Physical Form filled out (you can get these off the NSAA website or Norfolk Public Schools Website)
****Copy of your current shot record
****Copy of your insurance card (all visits will be run through insurance as a preventative service and should be paid by your insurance company)
****Parents can go on our website www.norfolkmedicalgroup.com, and print off all the necessary forms to decrease the wait time. The forms are
- PATIENT HISTORY/ASSESSMENT FORM
- PEDIATRIC REGISTRATION FORM
- PROTECTED HEALTH INFORMATION (PHI) FORM
