When you make an appointment, explain what your needs are so our staff can schedule an appointment of the appropriate length of time and with the appropriate provider. If you would like to request or change your primary care provider, we’ll do our best to meet your needs.
If you think you will be unable to make your appointment, then please call us immediately if you are unable to keep an appointment.
Our reception staff will ask you to fill out registration and health history forms. We ask that new patients arrive 30 minutes before scheduled appointment time to complete paperwork. These forms may be accessed on our website at www.www.norfolkmedicalgroup.com
Please bring all medications you are currently taking and your insurance card or copy of your card. We do not take digital copies of insurance cards, ie cannot be a picture on your cell phone. If your insurance requires a co-pay or deductible, please be prepared to pay that at the time of your visit. In addition, it is helpful to write down a list of questions for your provider so that we can ensure that we answer all of questions in the exam room.

We recommend that you call the office as soon as possible at 402-844-8000 to schedule, reschedule or call about your missed appointment. If you are a new patient, please call 402-844-8000 to schedule an appointment. We have many options for same day appointments.
We ask that you contact your pharmacy before your medication runs out to request a refill. Physicians may have to review your medical record before prescribing or refilling any medications. Please note that any new medications may require a patient visit and all controlled substances require a visit every three months. Any new prescriptions or refills can take up to 72 hours to complete. Please always be sure to call your pharmacy before your medication runs out.
Lab test and x-ray results may take up to 5 business days. If after 5 business days and you have not heard from our office either by phone call or letter, please call the office and speak with the phone nurse.
If you to schedule an MRI, Nuclear Medicine study or CT please allow our office 5 to 7 business days to obtain authorization from your insurance company. Insurance Companies require medical records from your physician and be reviewed by the Insurance Company’s medical staff before an authorization is obtained.
If you need a referral, please contact our office to see if you need to schedule an appointment with your physician prior to the referral being issued. Your insurance company may require that your Primary Care Provider has seen you prior to being sent to a Specialist. Our office number is 402-844-8000 to schedule an appointment.
Each insurance plan is very different; therefore, we recommend that you contact your insurance plan to determine if the anticipated services are a benefit of your individual plan. We are only able to provide quotes for the office visit portion of your visit. If the services are not covered, we can work with you to develop a payment plan.
The Patient Portal enables you to view your health records easily, safely and securely from the comfort of your home. The Patient Portal is used to print your medication list, your child’s immunizations, lab results, and access patient education. At the time of your first visit or any subsequent visits after you have established care, you will be given your individual unique patient number and be given instructions on how to log into the Patient Portal. You will need to use your email address to set up your Patient Portal.
Forms can be dropped off at the front desk. Depending on the type of forms, like FMLA, Disability and Life Insurance may take up to 5 days to complete. Some paperwork might require an appointment with your physician for completion.
